Since its inception in 1997, the OLHOF’s mission has been to recognize & celebrate the historical, cultural, and athletic contributions within the Ontario lacrosse community; as well as to provide educational information resources about the game and its rich history. The OLHOF celebrates & recognizes any individual who has made a significant contribution to the sport of lacrosse, or who has attained a high level of accomplishment in the game.

To confirm that an individual meets the criteria for a specific category, please complete and submit the relevant OLHOF Nomination Eligibility Form below. Once eligibility has been confirmed, a link to the nomination form will be sent to the nominator to complete the process. Successfully nominated candidates will be considered for induction the year following their nomination by the OLHOF Selection Committee.
