Criteria for Induction
Nominations
The OLHOF celebrates & recognizes any individual who has made a significant contribution to the sport of lacrosse, or who has attained a high level of accomplishment in the game.
To confirm that an individual meets the criteria for a specific category, please complete and submit the relevant OLHOF Nomination Eligibility Form below. Once eligibility has been confirmed, a link to the nomination form will be sent to the nominator to complete the process. Successfully nominated candidates will be considered for induction the year following their nomination by the OLHOF Selection Committee.
